What might your typical day look like?

• Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values!
  ‍
• Lead and motivate a team of Stop Smoking practitioners to work together and innovate to support clients.
  ‍
• Working with partner organisations and colleagues in Primary and Secondary care to diversify the current Stop Smoking pathway and improve client uptake and     meet targets by reaching new client groups.
  ‍
• Work towards targets, reflect and plan for areas that require improvement and be a driven lifelong learner in the field of Stop Smoking.
  ‍
• Develop and deliver updates and reports for wider team and Commissioners.
  ‍
• Monitor outcomes and performance to deliver on contracted KPIs.
  ‍
• Ensure an excellent and safe experience for residents using our policies and procedures.
  ‍
• Working with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments – we’re always looking to share knowledge and resources!
  ‍
• Ensuring due consideration is given to Information Governance and Clinical Governance legislation, requirements and guidance.

Contract: Permanent Contract, Full time 37.5 hours (Please note the current OYL contract ends June 2025).

Salary: Starting at £28,000 per year.
‍

Location: This job is a hybrid remote role based in Lincolnshire,with regular travel across the County (car owner required).
