Long Term Conditions Prevention Pilot - A success for Thrive Tribe
The Long Term Conditions Prevention Pilot (LTCP) is a 3-year NHS England initiative and newest component of the NHS Healthier You programme, designed to help adults who have been diagnosed with hypertension (high blood pressure) reduce their risk of developing serious long-term health conditions, including heart disease and stroke. Building on the success and learning of the NHS Diabetes Prevention Programme (NDPP), the initiative provides people with access to a group Health Coach supported 10 month programme to help them better manage their health and reduce the risk of future complications. Patients are identified through participating GP practices and invited directly to access the programme.
Thrive Tribe has been delivering the LTCP across two contracted areas, South East London (SEL) and North West London (NWL). Working closely with GP practices, NHS partners and commissioners, our teams have successfully mobilised and delivered the programme, coordinating patient referrals, engagement, onboarding and group-based support. Across Wave 1, our delivery has included establishing 32 face-to-face groups, alongside managing the referral pathway and supporting patients to access the most appropriate intervention. This has involved significant coordination, flexibility and commitment from colleagues across our operational and delivery teams.
A particularly significant achievement has been the recognition of our work in South East London, which NHS England highlighted as the best-performing LTCP area in the country. This is a fantastic endorsement of the work undertaken by the team and reflects the strength of our delivery, partnership working and approach to engaging patients in preventative healthcare. It is also a great example of how Thrive Tribe continues to make a difference by helping people take control of their health, preventing future illness and supporting the wider NHS ambition to shift towards prevention rather than treatment.

We are now looking forward to launching Wave 2 in January, which will see the programme extended to a further 24 GP practices, creating the potential to reach up to twice as many eligible patients and giving even more people the opportunity to access support.
