Gloji Smokefree City & Hackney are the smoking cessation service for The City of London and London Borough of Hackney. Residents across the are able to benefit from a variety of our established stop smoking pathways, including GlojiSmokefree, our online programme. As a Primary Care Liaison Officer, your role will be to support referral pathways from Primary Care into the service.
The Primary Care Liaison Officer aims to provide a dedicated point of contact in the organisation and an effective link between primary and secondary care. Our focus is to forge strong relationships with primary care colleagues and continually explore ways to build and improve these relationships to ensure the best care. What might your typical day look like?
· Working closely with primary care colleagues and partners on three key primary objectives (Delivery of all GlojiSmokefree City & Hackney (C&H) KPI’s, Referrals into the C&H stop smoking service and the recoding of current patient records where possible).
· Working towards a full C&HSMART Card coverage within every surgery within the City of London and London Borough of Hackney.
· Working with local surgeries, PCN’s& PPG groups to ensure the whole C&H service is promoted and marketed to a high standard
· Accessing GP patient lists to identify recorded smokers, offer support and code the outcome into the patient record
· Arranging Practice visits and events and ensuring GPs and practice teams are kept up to date with relevant news,building extensive relationships between primary care and C&H
· Making improvements in communications and services and creating opportunities for clinicians to work together for better patient care
· Working with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments – we’re always looking to share knowledge
and resources!
· Ensuring due consideration is given to Information Governance and Clinical Governance legislation, requirements and guidance

Contract: Permanent Contract,full time 37.5 hours.
Salary: Starting at £24-30,000 dependent on experience.

Location: This role is based in the City of London and London Borough of Hackney and will involve extensive travel across the area for meetings / partnership opportunities however there will also be some home working possible.
